This case report describes the impact of aquatic physical therapy on a patient who developed osteonecrosis of the right femoral head following a crisis caused by sickle cell disease (SCD). The patient reported pain during physical effort, reduced mobility, muscle weakness, and impaired coordination. The aim of this study was to evaluate the effects of aquatic physical therapy on hip muscle strength and mobility, as well as to observe changes in quality of life in a young individual with femoral head osteonecrosis. The patient underwent ten sessions of aquatic physical therapy focused on improving muscle strength and hip mobility to enhance functionality and quality of life. Muscle strength was assessed using the Lafayette dynamometer, mobility was evaluated through the Y Balance Test, and quality of life was measured using the SF-36 questionnaire. Post-intervention reassessment demonstrated remission of pain, increased strength in the adductor and abductor muscles of the right lower limb, achieving symmetry with the contralateral limb, and bilateral improvement in hip flexor strength. Mobility outcomes showed increased range of motion in all joint movements bilaterally. Quality of life improved across all domains evaluated by the questionnaire. In conclusion, aquatic physical therapy had a positive impact on the patient’s recovery and functional development, improving muscle strength, mobility, and overall quality of life.
